Incident Investigations
Meeka OHS Consulting specializes in incident investigations to find the basic and root causes so there is no repeat of the same incident. We follow a formal process which includes documenting and tracking investigations. When the investigation is done, we make recommendations to the management and safety committees to prevent a re-occurrence.
We also discuss the outcome with the staff in the specific area or department where the incident occurred. The Occupational Health and Safety Regulations (GAR9) stipulates clearly on why and when incidents need to be investigated. Incidents normally occur when a section of the safety management system fails. Near Miss events must be reported to the Department of Labor. OHSA Section 24 stipulates that you need to report certain incidents to the Chief Inspector, and GAR (General Administration Regulations) Section 8, stipulates that the Employer has to report incidents (within 7 days of incident) and occupational diseases (within 14 days).
